Guide d’accompagnementLes matrices emplois-expositions sont des outils d’évaluation des expositions professionnelles utilisées en épidémiologie des risques professionnels. Elles permettent d’attribuer automatiquement à des travailleurs par la simple connaissance de leurs emplois (profession occupée dans un secteur d’activité), une probabilité d’être exposés à des facteurs de risque présents dans l’environnement de travail. Le présent guide vise à présenter la matrice emplois-expositions aux pesticides développée dans le cadre du programme Matgéné. Les pesticides évalués dans cette matrice emplois-expositions sont : d’une part les produits phytopharmaceutiques (PPP) utilisés pour protéger les plantes des agressions de nuisibles, qui sont regroupés selon quatre sous-catégories (herbicides, insecticides, fongicides, autres) ; et d’autre part, les produits biocides qui sont utilisés pour protéger les humains et les animaux des nuisibles, définis selon trois catégories, les produits biocides (22 sous-catégories définissant des types de biocides), les médicaments à visée biocide (2 sous-catégories pour les médicaments humains et les médicaments vétérinaires) et les produits utilisés pour la désinfection des dispositifs médicaux. La matrice emplois-expositions aux pesticides a été construite par expertise suite à une importante recherche d’informations permettant de documenter les différentes situations d’exposition professionnelle pour l’ensemble des travailleurs en France. La matrice fournit une probabilité d’exposition (représentant le pourcentage de travailleurs dans l’emploi jugés exposés) pour chacune des sous-catégories et des catégories de pesticides, ainsi qu’une évaluation globale à tous pesticides, pour tous les emplois jugés exposés.
17/09/2026 00:00 Öffentliche Gesundheit Frankreich Environmental burden of lead (Pb) and methylmercury (MeHg) exposure on IQ loss in children in Europe - single substance and mixture approachLead (Pb) and methylmercury (MeHg) are well-established developmental neurotoxicants, associated with adverse effects on cognition in children. This study estimates the environmental burden of disease associated with Pb and MeHg on IQ loss of children. Human biomonitoring surveys from five European countries were used to estimate exposure for children and women of childbearing age, covering all exposure sources and routes. Coupled with exposure-response functions we derived from meta-analyses, IQ points lost attributable to each metal were estimated. Assuming a normal distribution of IQ points, we calculated the resulting IQ score due to exposure and applied disability weights to calculate DALYs. Uncertainty (on model parameters) and intra-populational variability (exposure differences) were propagated through Monte Carlo simulations. The burden due to combined exposure to both substances was estimated for the French dataset, which met data requirements for a mixture assessment. The estimated IQ shift between exposed and theoretically unexposed individuals was higher for Pb than for MeHg. Pb was responsible for a median shift in IQ points of -0.36 (Sweden) to -0.47 (Czech Republic), while MeHg contributed to -0.03 (Czech Republic) to -0.08 (France) IQ points. We estimated a mean attributable burden from 950 to 1173 DALYs per 100,000 individuals for Pb, and 96 to 251 DALYs per 100,000 for MeHg across the studied countries. The burden of combined exposure in the French population was estimated at an average of -0.49 IQ points lost per individual, and 1,367 DALYs per 100,000 births. Our results highlight the necessity to strengthen regulatory measures to reduce lead exposure through key sources such as dust, tap water, and food, despite decreasing blood levels in Europe in the last few decades. Regarding MeHg, our study highlights the importance of maintaining targeted communication towards current and future parents, especially because dietary exposure remains the main exposure source.
15/09/2026 00:00 Öffentliche Gesundheit Frankreich L’exposition professionnelle aux produits phytopharmaceutiques en 2019 en France et son évolution depuis 2007Introduction – L’exposition professionnelle aux produits phytopharmaceutiques (PPP) peut avoir des effets nocifs sur la santé (par exemple : maladie de Parkinson, hémopathies malignes, cancer de la prostate…). L’objectif de cette étude était de déterminer les proportions et les nombres de travailleurs exposés aux PPP, de façon exhaustive, sur la période 2007-2019 et de les décliner suivant différents critères (statut du travailleur, secteur d’activité…).Méthode – Une matrice emplois-expositions spécifique des PPP (classés en quatre catégories : herbicides, insecticides, fongicides et autres) a été développée par expertise dans le cadre du programme Matgéné. Elle a été ensuite croisée avec les données des recensements de la population de 2007, 2013 et 2019. Les nombres et proportions de travailleurs exposés, ainsi que leurs intervalles de sensibilité (IS) ont ainsi pu être estimés.Résultats – En 2019, 1,7% (intervalle de sensibilité, IS: [1,5-2,0]) des actifs en emploi en France hexagonale, âgés de 20 à 74 ans, étaient exposés à au moins une catégorie de PPP, représentant 449 260 travailleurs [381 000-514 020]. Les non-salariés avaient une proportion de personnes exposées plus élevée que les salariés (9,0% [7,9-10,1] versus 0,7% [0,6-0,9]). Les personnes exposées travaillaient très majoritairement dans l’agriculture (90%), et dans une moindre mesure, dans le tertiaire, notamment en tant que jardiniers.Discussion – Ces indicateurs sont les premiers documentant l’exposition aux produits phytosanitaires pour l’ensemble de la population au travail, quel que soit le secteur d’activité, et sur l’ensemble de la France hexagonale. Ils confirment la forte proportion de travailleurs exposés aux PPP dans le milieu agricole, mais mettent aussi en lumière les emplois non agricoles. Ces résultats pourront être utilisés par les préventeurs pour orienter leurs actions.
15/09/2026 00:00 Öffentliche Gesundheit Frankreich Relevance and implementation of human biomonitoring to assess the chemical exposures following an industrial accidentBACKGROUND: After an industrial accident, workers at the site of the accident, as well as workers from nearby companies and responders (such as firefighters and police officers), and the surrounding population could be exposed to hazardous chemicals. In these situations, human biomonitoring measures are required by the population to assess the impact of the accident. However, HBM studies are usually conducted to assess the chronic exposure of chemicals for the general population, and no frame exists to apply these measures to such accidental context. OBJECTIVE: Following a major industrial fire that occurred in Normandy in 2019, Santé publique France established a framework for HBM measurements following an industrial accident, in order to assess the relevance of such measurements and give operational guidelines to carry out these measures, from the sampling to the interpretation of the measures, in support to policy makers and health professionals. METHODS: A bibliographic search on HBM protocols in case of accidents and HBM studies already implemented worldwide following an accident was conducted as well as numerous consultations covering scientific aspects, ethical and deontological considerations, and practical aspects of implementing an HBM protocol in the event of an accident. RESULTS: The HBM framework designed includes an initial HBM phase within 48 h following the end of emissions from the accident. Given the short deadlines for this first phase, it is advisable to target the most exposed and impacted individuals, that is, all persons (workers, occupants, emergency services, etc.) who were present on site during the industrial accident, as well as all individuals who visit a healthcare facility from the day of the accident until 48 h after the end of accidental emissions and for circumstances related to the accident. Sampling kits have to be quickly deployed in the event of an industrial accident requiring the implementation of HBM. Following this initial phase, medium- and long-term biological monitoring may be considered based on available information about the emitted chemicals and observed environmental contamination. IMPACT STATEMENT: The assessment of exposure to chemical substances through HBM after an accident requires the development of a standardized framework, particularly for chemicals with short half-lives. Exposure to chemical substances, after an accident, affects the general population residing near the accident site, as well as workers and emergency services.
